Initial Appointments

All appointments with our Surgeons require a written referral from your GP or Specialist.

 

Please forward us your referral so that it can be triaged to ensure urgent cases are seen promptly.

Allow 5 working days, then call us on 02 6021 7111 between 9:30am to 4pm Monday to Friday to make your appointment. 

HELP US STAY COVID SAFE:

If you answer YES to one or more of the following questions, DO NOT attend our rooms, please contact us to discuss the alternatives regarding your appointment:

   

  • Are you UNVACCINATED (i.e. NOT DOUBLE VACCINATED) against COVID-19

  • Have you recently tested positive to COVID-19 

  • Have you or a member of your immediate household had, or been in contact with someone who has had, a confirmed case of COVID-19 in the last 14 days?

  • Do you currently have any flu like or respiratory symptoms – fever, cough, sore throat, shortness of breath, loss of smell or loss of taste?

  • Having reviewed the list of the latest COVID-19 case locations in NSW and other states, have you been to a venue on a date and time that requires self-isolation?

Please arrive at our rooms no earlier than 15 minutes prior to your scheduled appointment as we have limited seating capacity due to COVID restrictions.

Prior to your Appointment

Please forward a copy of your referral with all relevant test results to:

SMS reminders

You will receive an SMS text to your mobile 1 to 3 days prior to your appointment, reminding you of your consultation date, location and time.

What to bring to your Appointment

  • PROOF of vaccination against COVID-19 (without proof of vaccination you will not be granted access to our practice)

  • Wear a MASK. (Proof for a medical exemption is required, e.g. a medical certificate or letter signed by a registered health practitioner, such as a doctor, or a registered NDIS provider)

  • Medicare Card

  • Veterans Affairs Card (DVA)

  • Private Health Insurance Card

  • Workers Compensation details - claim number, insurer and case manager contact details

  • Relevant X-ray films or Reports 

  • list of your current medications and allergies

  • List of questions you may have

On arrival you will be required to check into our practice using the QR code provided via the NSW service App. If you do not have this app, one of our staff can check you in.

 

All new patients are asked to complete our patient registration and privacy consent form, however due to COVID-19 restrictions, our staff will obtain a verbal consent from you.  Alternatively you can download the patient registration and privacy consent form below and bring the completed form with you to your appointment.
