Patient request for access to Medical Records 

Following all consultations and surgery, correspondence is sent back to your referring doctor (and additional medical practitioners involved in your care as requested) by way of consultation letters, operations reports and/or results to keep them informed of your progress. Your general practitioner is the first point of contact regarding access to your medical file as they are usually your referring doctor and the primary practitioner involved your care. 

Albury Surgical Group provides patient access to medical records in accordance with the Privacy Act and Australian Privacy Principle (APP) 12 which deals with access to personal information. 

  • We can electronically send your medical record to your new GP or specialist. Your new practitioner must forward us a request in writing with your signed consent.

  • As a patient, you may request a copy of your medical file. Your request must be in writing and you must provide proof of identity prior to the request being authorised.

  • The minimum administrative fee charged to patients is $110 Inc GST. Payment is required prior to medical records being released. The fee may be higher depending on the size of the record and if the file has to be retrieved from the archives of a previous ASG surgeon who has retired or moved.

  • The request will be handled within 45 days of the written request being received and/or within 7 days of payment being received.
